NEWLY HIRED EMPLOYEES
(Employees who were not eligible for dental coverage under the Trust
in the prior month) |
Coverage begins when the
5th consecutive monthly employer contribution is made and
is retroactive to the first day of the month for which the
employer's 1st contribution was due and paid. |
NEW BARGAINING UNITS
(Employees in a bargaining unit of an existing participating
employer or of a newly participating employer that is becoming
covered for dental under the Trust as part of a new unit of
employees.) |
Individuals in the new bargaining-unit who
are employed on the effective date of the unit's participation
that meet the hours requirement (usually 80) for the first
month, will be eligible on the fist of the month in which
the initial payment is due and paid. |
CONTINUING EMPLOYEES
(Employees who were eligible for dental coverage under
the Trust in the prior month) |
Eligibility is provided
in the month following the month in which the required number
of hours (usually 80) were worked/compensated and an employer
contribution is paid. |
FORMER
ELIGIBLE EMPLOYEES
(Former eligible employees who have not been eligible for dental
coverage under the Trust for five consecutive months. |
Must meet the "Newly Hired
Employees" eligibility criteria stated above.
